Benefits & Offers
- The Discount Card Shall be issued @Rs.500/-, the Reward Card @Rs.1000/- & the Advantage card @Rs.5000/- with preloaded benefit of RE. O, Rs. 1200, RS. 6300/- respectively.
- Reward Card & Advantage Card member also get one complimentary test for any one member in the family. The reward card member get one sugar PP or Fasting Test worth Rs.70/-complimentary & Advantage Card member get either Lipid Profile or LFT or Thyroid Profile or HbA1C test worth Rs. 700/-complimentary for any one member in the family.
- The cardholder will be entitled to a discount of 10% on routine lab investigation except some Special cases & 5% discount on Radiology, CT scan & MRI (except contrast) & additional 5% discount to senior Citizen
Term & Conditions
- Benefits will be offered only to the card holder & their family members mentioned at the time of registration.
- The Card shall be valid for the period of 12 months from the date of issue & can be used with immediate effect.
- Benefits of Rs. 1200/- can be availed on minimum billing of Rs. 500/- with a cap of Rs. 300/- (300 x 4 = 1200) & benefits of Rs. 6300/- can be availed on minimum billing of Rs. 1500/- with a cap of Rs. 1050/- (1050 x 6=6300) till you exhaust the entire balance. Any test below the minimum slab amount shall get the flat discount mentioned.
- The wallet balance forfeits if remain unused upon expiry. In exceptional cases if any test is claimed for refund, the balance used from the wallet will be conditional.
- To redeem the wallet balance as per schemes, you must produce the card at the time of billing or final settlement.
- Offers & Benefits cannot be clubbed with any other offers, promotions or discount.
- The Physical Card shall be issued within 1-5 working days of billing.
- In the event of any dispute in interpretation of the Terms & Conditions, governing the schemes, the decision of the management will be final & binding.
